Registering with the Practice. 

In order to register with the practice you will need one of the following:
  • Northern Ireland Medical Card

  • HS200 registration form is for registering patients within the UK.

  • HS22X registration form is for registering patients from outside the UK.              

  • All patients need to bring photo ID

  • A list of any medications you are taking - given to you by the practice you are coming from.

When registering small children you need to bring their red book, it is important the practice has an up-to-date vaccinations history of your child.

Patients will be registered as temporary until blood pressure etc have been recorded.  Failure to have this taken your registration will not go through.

Once registered a medical card will be sent from Central Services Belfast - this can take between 4 - 6 weeks.
